[UPDATED] Are you a Returning Baguio Resident or Worker? Follow These Updated Guidelines



The City Government of Baguio has released updated guidelines for Returning Baguio Residents (RBRs) and Workers incoming and outgoing the City of Baguio while it is under General Community Quarantine (GCQ).

All inbound travelers are mandated to register their Health Declaration Forms (HDF) at http://hdf.baguio.gov.ph/. Here are the following documents needed and shall be uploaded to the traveler’s HDF profile:

If you are:

A. A resident of Baguio currently stranded in other provinces due to ECQ and is one of the following:

(Residente ng Baguio at nastranded sa ibang lugar o probinsya dahil sa ECQ at isa sa mga sumusunod:)

  1. Student
  2. Tourist or Visitor in another province and was stranded due to ECQ
  3. Worker or Employee in other provinces and is now going home to Baguio

Required Documents:

  1. Travel Authority from PNP Regional Director from the place of origin.
  2. Government ID or Barangay Certificate as proof that you are a resident of Baguio
  3. A negative result of RT-PCR or Rapid Antigen test conducted within 72 hours prior to entry in the City

Note: May opt to undergo a 14 day home quarantine instead of testing.

Process

  1. Fill-up the Health Declaration Form online via http://hdf.baguio.gov.ph/
  2. Upload all the required documents
  3. Undergo health check at the Central Triage Unit

B. A non-resident of Baguio currently employed in an establishment here in Baguio that was temporarily closed due to ECQ but is now asked to return to work

(Residente ng ibang lugar, at trabahador o empleyado sa isang establisimyento sa Baguio na pansamantalang pinasara dahil sa ECQ at ngayon ay pinabalik na sa trabaho)

  1. Workers in Baguio and living in nearby provinces, who regularly pass through checkpoints

Note: For this purpose, BLISTT residents shall follow the same required documents and process.

Required Documents:

  1. Valid Government Issued ID showing place of residence
  2. If passing through the checkpoints for purposes of work, business, or employment, any document as proof of APOR status (Employee ID, Certificate of Employment, Business Permit, IBP/PRC ID, Contract of Work, and etc.)
  3. Must present a negative result of RT-PCR or Rapid Antigen test conducted within the last 30 days prior to entry

Note: In the absence of negative test results, present your Medical Certificate issued by the Local Health Office from point of origin stating that the person is not a suspect, probable, or possible COVID-19 case before proceeding to the Central Triage for testing.

Process:

  1. Fill up Health Declaration Form (HDF) at http://hdf.baguio.gov.ph/
  2. Undergo health check at Triage 1 on your return
  3. Show documents at the checkpoint

C. A resident of Baguio who needs to go out for essential travel such as:

(Residente ng Baguio at kinakailangan lumabas ng pansamantala dahil sa essential travel)

  1. Procurement or delivery of PPE materials to other places
  2. Fetching of personal possessions that were left in their boarding houses outside Baguio
  3. Appointments with public or private agencies outside Baguio
  4. Medical appointments outside of Baguio

Required Documents:

  1. Clearance Certificate from the City Mayor’s Office
  2. Any proof of your essential travel purpose going to other places
  3. If you have a medical appointment outside of Baguio, bring a Doctor’s Referral

Process:

  1. Fill up Health Declaration Form (HDF) at http://hdf.baguio.gov.ph/
  2. Undergo health check at Triage 1 on your return
  3. Register in your corresponding barangay

NOTE:
1. Apply for Travel Authority two (2) days before your planned trip
2. All individuals with Indispensable (Essential) Travels (IT) should register online for contact tracing purposes at https://hdf.baguio.gov.ph/ (No need to wait for confirmation from CDRRMO if leaving Baguio)

C-B. Indispensable (Essential)Travels (IT) going out of Baguio for the following reasons:

(UPDATE AS OF JUNE 30, 2020)

  1. Family Emergency
  2. Medical Emergency
  3. Humanitarian Reasons

Required Documents:

  1. Barangay Endorsement from the office of the Association of Barangay Council (ABC) at the Baguio City Hall or at the one-stop-shop at PFVR gym
  2. Health Certificate from any of the District Health Centers or at the one-stop-shop at PFVR gym
  3. Travel Authority from BCPO stationed at the PFVR gymnasium, Military Cut-off Road

NOTE:
1. Apply for Travel Authority two (2) days before your planned trip
2. All individuals with Indispensable (Essential) Travels (IT) should register online for contact tracing purposes at https://hdf.baguio.gov.ph/ (No need to wait for confirmation from CDRRMO if leaving Baguio)

UPDATE AS OF JUNE 29, 2020: There is a New Venue for Processing of Travel Authority for Indispensable (Essential) Trips

D. A resident of Baguio and you need to go out in order to fetch:

(Residente ng Baguio at kinakailangan lumabas para manundo ng:)

  1. A Baguio resident stranded in other places.
  2. Non-OFW who came from another country.

Required Documents

  1. Clearance Certificate from the City Mayor’s Office
  2. Show the travel order, ID and the schedule of return of the person you are going to fetch.
  • For those fetching non-OFW residents of Baguio, the required documents are travel itinerary and ID as proof that the person is a resident of Baguio

Process

  1. Undergo health check at the Central Triage unit based on your arrival time.

E. A Resident from other provinces who needs to enter Baguio because of essential travel such as:

(Residente ng ibang lugar at kailangan pumasok sa Baguio dahil sa essential travel gaya ng)

  1. Get properties/things from their boarding house in Baguio.
  2. Appointment with a private or public agency in Baguio
  3. Medical appointment in Baguio City

Required Documents

  1. Travel clearance certificate from the Mayor or designated representative from the place of origin.
  2. Proof of your essential travel to Baguio City
  3. For those with medical appointments, bring your doctors’ referral.

Process

  1. Fill-up the Health Declaration Form online via http://hdf.baguio.gov.ph/
  2. Show documents at the checkpoint 
  3. Undergo health check at Triage 1.

F. Authorize Personnel Outside of Residence (APOR) with Official Travel order such as the following:

  1. Health workers
  2. Uniformed personnel
  3. Government Officials

Required Documents:

  1. Medical Certificate issued by the Local Health Officer at the place of origin
  2. Valid Government issued ID, and any document as proof of residence or assignment in the BLISTT (Baguio, La Trinidad, Itogon, Sablan, Tublay, and Tuba) area.

Process:

  1. Fill up Health Declaration Form (HDF) at http://hdf.baguio.gov.ph/
  2. Present your documents at the checkpoint
  3. Undergo health check at Triage 1

G. Employee of an establishment here in Baguio City or other places who needs to constantly go in and out of Baguio City due to nature of work

(Empleyado ng isang establisyemento sa Baguio o ibang lugar at kailangang maglabas-masok sa Baguio dahil sa uri ng trabaho)

  1. Employees of the public and private sector who are residents of Baguio and working in nearby provinces, and who regularly pass through checkpoints

Note: For this purpose, BLISTT residents shall follow the same required documents and process.

Required Documents:

  1. Medical Certificate issued by Local Health Officer at place of origin
  2. Valid Government Issued ID showing place of residence

Additional Reminders

  1. Everyone who will be entering Baguio City must submit themselves for a health check at Central Triage located at the checkpoints, before proceeding further. 
  2. Aside from the Baguio City Health Service and the 16 health centers in the city, there are two other additional offices that will issue Health Certificates:
    • Baguio City Hall for those applying for a Clearance Certificate from the City Mayor’s office
    • Baguio Athletic Bowl for those applying for Travel Authority from the PNP.

  4. What falls under proof of residency po? Pwede bang birth certificate? I used to study there a year ago but I don’t have my old IDs. All I have is my Good Moral Character from my old school and my birth certificate that says I was born there.

    I am a director of Highland Rural Bank in La Trinidad.
    I have to go home and be with my mother and update with my work at the bank.
