Baguio City Health Services Office Announces COVID-19 Vaccination Pre-Registration for Senior Citizens



The Public Information Office of the City of Baguio and the Baguio City Health Services Office has released an announcement on Monday, February 22, 2021, informing that senior citizens are included in the Phase 1 priority group to be vaccinated against COVID-19 and the vaccination form can now be filled out. The offices also emphasized in the announcement that the pre-registration will be closed on February 28, 2021.

Baguio Vaccination Form for Senior Citizens

Moreover, earlier today (Tuesday), February 23, 2021, the PIO Baguio and the city’s Health Services Office has published another advisory and here are some of the details indicated:

“For those senior citizens (59 y/o that will be turning to 60 y/o this year and above) residing in Baguio, you are advised to either pre-register for COVID-19 vaccination by submitting a Google Form Response or subjecting yourselves to interview by circulating BHWs or employees of catering DHCs with respect to their current address.”

Here is the Google Form link for Senior Citizens called the “Baguio Vaccination Data Collection Form: Senior Citizens”: https://forms.gle/XKBKxXSmKNcEgo5K6 as provided by the abovementioned offices.

Profile/Master List of Senior Citizens

Through the same advisory, all Baguio public and private hospitals, public and private long-term care facilities catering to senior citizens, District Health Centers (DHCs), Barangay Health Workers (BHWs), BHERTS and BNS that are active in service, were also asked to submit the profile/master list of senior citizens in their respective barangay.

Thus, the PIO Baguio and the city’s Health Services Office also clarified that “the Profiling & Master List is DIFFERENT from the Registration for vaccinees.”

Baguio Vaccination Form for Uniformed Personnel

Meanwhile, on a separate advisory which was also made public by the Public Information Office of the City of Baguio and the Baguio City Health Services Office today, it was announced that all uniformed personnel stationed in Baguio City (BJMP, PNP, BFP, AFP) are advised to pre-register for COVID-19 vaccination through a Google Form.
